Electronic Health Records (EHRs) are the standard for healthcare documentation — from hospitals to primary care offices to specialty clinics. Physicians and hospitals, however, have a wide array of choices when it comes to their EHR product, which can lead to patient information being stored in many different systems, resulting in fragmented care. Therefore, CHI has partnered with Monterey County hospitals to provide a Health Information Exchange (HIE) to our medical community. An HIE integrates patient data from different health information systems – creating a secure, unified patient record that is accessible to participating healthcare providers.
Our HIE provides patients and the healthcare community with a trusted, integrated, and efficient infrastructure to exchange health information in a way that facilitates high-quality, patient-centered care and overall population health. For example, hospital laboratory and radiology results can be securely and immediately delivered via our HIE to a patient’s primary care physician, and directly into their EHR system. Additionally, primary care providers can be immediately notified when their patients are discharged from the hospital’s emergency department, ensuring timely and safer transitions of care.
