Monterey PrimeCare Medical Group becomes Monterey County’s first level-3, patient-centered medical home

MONTEREY, CA – Monterey PrimeCare Medical Group, in partnership with Cypress Healthcare Partners (CHP) and Community Health Innovations (CHI), has been recognized as a level 3, patient-centered medical home — the highest distinction attainable by a medical practice — by the National Council Quality Assurance (NCQA). PrimeCare is the only medical group in Monterey County to achieve this prestigious achievement. NCQA’s patient-centered medical home program recognizes medical groups that demonstrate a patient-centric model that replaces single episodes of care with a coordinated approach.

“Our practice’s transformation to the patient-centered medical home care model — focused on the patient’s long-term health and wellness — supports our overall goal of helping residents of Monterey County live longer and healthier lives,” said PrimeCare provider Dr. Rebecca Abundis. “We believe this patient-focused approach is essential to creating strong relationships with our patients whereby together our care team can support their efforts to stay healthy and prevent illnesses from occurring.”

NCQA’s program is an innovative approach to primary care, where team-based care is organized around the patient and measured over time. Teams monitor health progress, track clinical results and health metrics, develop personalized care plans, and coordinate community resources, ensuring that patients receive the right care at the right time. To achieve the highest level of recognition, Level 3, practices must demonstrate and submit information on a rigorous set of NCQA standards.

“Level 3 recognition is a significant achievement for PrimeCare and our community because it engages patients in managing their health with a shared goal of avoiding preventable illness, managing chronic disease, and helping them make informed decisions for staying as healthy as possible,” said Liz Lorenzi, CHI’s chief operating officer. “Becoming a patient-centric practice requires a dedicated team of individuals to re-engineer workflows, expand access to care, and redefine the roles of the care team.”

CHI engaged CHP, a Monterey-based practice management company, as a strategic partner in the PCMH transformation work.   Together the organizations established the Best-Practice Team (BPT), a collaborative of experienced practice operations staff and nurses to design and implement the new workflow processes. “Patient-Centered Medical Home will be a cornerstone of PrimeCare’s integrated population management strategy,” says Bill Hines, president of Cypress. “It requires a different approach than health systems use today. Health care is migrating away from episodic care—where people go to the doctor only when they’re sick—toward preventive care, as we address new ways to engage the patient, the family and caregivers in taking ownership of their health.” CHP, a Monterey-based practice management company, led the transformation and recognition process for PrimeCare. CHP has a team of NCQA patient-centered medical home-certified content experts. Additionally, CHI provides the dedicated nurse care manager to PrimeCare, an essential component of a patient-centered medical home care team.
