Community Health Innovations appoints chief medical officer

MONTEREY, CA – Coordinated, patient-centered care will be the focus for Dr. Anthony Chavis, the new chief medical officer for Community Health Innovations (CHI), a subsidiary of Community Hospital Foundation, the nonprofit parent company of Community Hospital of the Monterey Peninsula.

CHI is focused on leading the transformation of healthcare in Monterey County. In partnership with doctors and other healthcare providers, CHI is introducing a concept called Integrated Population Management (IPM). It’s a patient-centered approach that brings together hospitals, doctors, and other providers to coordinate care, manage disease, and promote wellness.

Chavis spent the past 12 years as the vice president of Medical Affairs for Community Hospital, where he was responsible for the quality and safety of patient care “I want to recognize the extraordinary work of Dr. Anthony Chavis,” said Dr. Steven Packer, CEO of Community Hospital of the Monterey Peninsula. “For the past 12 years, Dr. Chavis has brought deep insight and intellect, an extraordinary work ethic, and great humanity to the role of vice president of Medical Affairs.
