MONTEREY, CA – Julie Da Silva, care manager team lead for Community Health Innovations (CH), a subsidiary of Community Hospital Foundation, is Monterey County’s first certified preceptor under the Geisinger/xG Health Solutions™ Advanced Case Management training program.
This certification is critical to building a strong team of local case managers who partner with healthcare providers to provide accessible, comprehensive, and coordinated care for patients. CHI has partnered with xG Health Solutions™, powered by Geisinger Health System, to focus on delivering the best healthcare possible to patients in Monterey County. By focusing on a model known as the patient-centered medical home (PCMH), the patient becomes the center of an effort that includes an entire healthcare team, advanced health information technology, and evidence-based medicine.
